Lehigh Acres carnival ride accident report released

LEHIGH ACRES, Fla. - Investigators for the Florida Department of Agriculture and Consumer Services released a report into a carnival ride accident at the Lehigh Acres Spring Fling that injured three people, all children.  The report raises a number of concerns surrounding the ride.

The rotating ride called the Ring of Fire came to an abrupt stop the night of Tuesday March 19, 2013.  The failure caused damage to the ring portion of the ride and left several sections bent and hanging out.

The inspectors discovered the following.  A witness had seen two caster wheels fall off the ride two days before.  That the Ring of Fire was originally permitted in October 2004.  Investigators discovered that the ride had been in an accident a few years after that and declared a total loss by the insurance company.

Modern Midways which currently operates the ride had it permitted in October 2011.  Investigators found that at 23 different locations, inspectors recorded problems with the ride including cracks to the inertia ring, problems with the safety cables, missing wheels and missing fastener nuts.

The last inspection that found problems was on January 25, 2013 when the ride was shutdown for several days due to deficiencies. 

In the end investigators could not determine the specific cause of the accident because the ride was so severely damaged.
